Same way every time

The four hundredth time is the first time.

How I use it

Once the local procedure is the one that keeps people safe, I run it the same way on the four hundredth repetition as on the first. I write what actually happened, not what was supposed to happen. I raise something out of tolerance instead of hoping it holds. Fresh eyes are for inventing the procedure. Discipline is for running it.

What I refuse

I know this one. We can skip the step. It will probably hold.
